HealthDay - TUESDAY, May 4 (HealthDay News) -- A quick follow-up visit by a medical professional to a heart failure patient discharged from the hospital reduces the chance that the patient will wind up back in the hospital, but that preventive measure is more often ignored than observed, a new study finds.
